 Chronic Disease Self-Management Services

As a Health Psychologist and Counsellor, I take an individualised approach to supporting chronic conditions self-management. Some of the techniques I might use include, cognitive-behavioural therapy, mindfulness, and behaviour change strategies. At the core of all my work with clients is a positive psychology approach, where we work together to harness the skills and abilities you have – to make your life easier and improve your quality of life.

6-Week Self-Management Programme for Chronic Conditions

Format: Skype

The 6-week self-management programme is holistic, person-centred, and tailored to meet your specific needs. It comprises 1.5 hours of one-to-one work per week, with self-management activities between sessions. Email support is provided between sessions during the 6-week programme.

Subjects covered include:

1) Self-management skills (including appropriate use of medication, accessing support), and confidence to self-manage.

2) Emotional well-being, including techniques to deal with problems such as low mood and self-esteem, anxiety, or frustration.

3) Cognitive well-being, including decision-making, goal setting, and action planning.

4) Physical well-being, including symptom management

5) Social well-being, including isolation and communicating effectively with family, friends, and health professionals.

6) Lifestyle, including meeting your nutritional and physical activity needs.

Each programme will be tailored to the chronic condition you are living with (i.e. diabetes, arthritis, pain, etc.) and then personalised further to your unique needs (based on initial discussion and assessment). Emotional and cognitive needs will be addressed first in order to build your confidence to approach the physical and social aspects of living with your chronic condition.

Weekly action plans will be completed, along with discussion of any problems encountered and how they were or can be overcome.

Single Session Self-Management Consultancy for Chronic Conditions

Format: Skype

Individual 1-2 hour one-to-one sessions can be provided on a selected are of concern, such as:

Subjects covered include:

1) Self-management skills (including appropriate use of medication, accessing support), and confidence to self-manage.

2) Emotional well-being, including techniques to deal with problems such as low mood and self-esteem, anxiety, or frustration.

3) Cognitive well-being, including decision-making, goal setting, and action planning.

4) Physical well-being, including symptom management

5) Social well-being, including isolation and communicating effectively with family, friends, and health professionals.

6) Lifestyle, including meeting your nutritional and physical activity needs. 

Types of Chronic Conditions

All types of chronic conditions can be assisted by the 6-week programme or single sessions. Specific areas addressed according to condition, and based on individual needs, might be as follows: 

Chronic Pain

In addition to the subjects covered in the self-management programme and single sessions, the following components might also be included if appropriate to your needs:

  • Techniques to deal with depression and anxiety, as well as fear of pain or injury.
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance.
  • Making decisions about treatment and complementary therapies.
  • Working with your pain.
  • Pacing activity and rest.
  • Breathing, meditation, and mindfulness to reduce stress.
  • Monitoring your pain 

Diabetes

In addition to the subjects covered in the self-management programme and single sessions, the following components might also be included if appropriate to your needs:

  • Techniques to deal with depression, anxiety, and stress.
  • Blood glucose monitoring.
  • Managing diabetes-specific symptoms such as hyper/hypoglycaemia, fatigue, visual problems, foot care, and Lipohypertrophy, etc.
  • Injecting yourself.
  • Weight and nutrition. 

Arthritis

In addition to the subjects covered in the self-management programme and single sessions, the following components might also be included if appropriate to your needs:

  • Techniques to deal with depression, anxiety, and stress.
  • Managing arthritis-specific symptoms such as pain, stiffness, inflammation, and swelling.
  • Physical activity to strengthen muscles and build flexibility and stamina.
  • Working with your limitations.
  • Nutrition for strength.
  • Getting a good night’s sleep. 

Cancer Survivorship

In addition to the subjects covered in the self-management programme and single sessions, the following components might also be included if appropriate to your needs:

  • Techniques to deal with problems such as living with uncertainty, fear of recurrence, etc.
  • Making decisions about treatment and complementary therapies.
  • Finding a new ‘normal.’
  • Dealing with changes in relationships.
  • Return to work.
  • Self-identity and body issues.
  • Managing treatment-related symptoms such as lymphedema, impotence, fatigue, loss of concentration, etc.
  • Self-examination.
